If you are less than 18 years of age, you must be accompanied by a parent or guardian. Please contact the immunization coordinator (260-499-4182, ext. 1) for more information.<br \/> <\/span><\/p>\n<p class=\"p2\"><span class=\"s3\">Vaccines (<a href=\"http:\/\/www.cdc.gov\/spanish\/inmunizacion\/index.html\"><span class=\"s4\">en Espa\u0144ol<\/span><\/a>) are given based on guidelines from the <span style=\"color: #0ca5cc;\">Indiana<\/span> <a href=\"http:\/\/www.in.gov\/isdh\/25720.htm\"><span class=\"s4\">Department of Health (IDOH)<\/span><\/a>\u00a0and the\u00a0<a href=\"https:\/\/www.cdc.gov\/vaccines\/schedules\/index.html\"><span class=\"s4\">Centers for Disease Control and Prevention (CDC)<\/span><\/a>\u00a0schedule.<\/span><\/p>\n<p class=\"p2\"><span class=\"s2\"><a href=\"https:\/\/www.in.gov\/isdh\/26550.htm\">Lead Poisoning Prevention Program<\/a><\/span><\/p>\n<p class=\"p6\"><span class=\"s1\"><b>For Children:<\/b><\/span><\/p>\n<p>To qualify for the free vaccines you must:<\/p>\n<ol>\n<li>Have no medical insurance. The purpose of CHIRP is to collect, forecast, manage, and share vaccine data to registered medical providers in Indiana. A provider may include private medical facilities, local health departments, hospitals, schools, and pharmacies. The program is run under the Indiana Code 16-38-5 which allows for providers and their designees to access data from CHIRP. In addition an individual also may access their personal or their dependents vaccine record from either the registered CHIRP provider or directly from the Indiana State Department of Health. Be the child\u2019s parent or have proof that you are the child\u2019s legal guardian OR You are at least 18 years old and have a note from the child\u2019s parent or legal guardian, giving you permission to bring the child. A note is needed for every visit.<br \/> 2. Permission forms must be completed each time you visit the Clinic.<br \/> 3. Child\u2019s immunization history is required every visit.<br \/> 4. Child must not have a croupy cough, fever, vomiting, or diarrhea in the past 24 hours.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">Tuberculosis Test (TB test)<br \/> The LaGrange County Health Department offers tuberculosis (TB) skin testing on Monday from 8:30 am to 3:30 pm at the main office. Cost is $20.00 for the test, and it\u2019s read on Wednesday. The nursing office provides extensive case management services to residents diagnosed with active or latent TB. Children who are on Medicaid, have no medical insurance, or who have insurance that does not cover vaccinations will be free of charge. All other patients will be charged $25.
